College Prep Form and Instructions This College Prep form is required for freshmen applicants in Ohio.
The student either meets the College Prep Curriculum, OR the ACG Grant Curriculum, OR none.

College Prep Curriculum
- 4 units of English
- 3 units of Math (including Algebra I, Geometry, Algebra II, Trigonometry, Pre-Calculus or equivelant courses)
- 3 units of Social Studies
- 3 units of Science
- 2 units of Foreign Language (The two units must be in the same language. American Sign Language is included.)
ACG Grant Curriculum
- 4 units of English
- 3 units of Math (including Algebra I, Geometry, Algebra II, Trigonometry, Pre-Calculus or equivelant courses)
- 3 units of Social Studies
- 3 units of Science (at least two courses that include a lab; including Physics, and either Biology or Chemistry)
- 1 unit of a Foreign Language
