Certificate of Insurance
A certificate of insurance validates the existence of an insurance policy. It may be issued by an issuer or an insurance agent or broker. A certificate of insurance should include the following:
• Name of the insurance company and policy number
• Policy period
• Name of the insured and address
• Description of coverage
• Description and location of operation
• Name and address of certificate holder
• Policy limits
• Notice of cancellation provision
• Authorized signature and date
Certificate of Insurance Requests
University groups using non-University facilities are often required to furnish a certificate of insurance evidencing the existence of insurance coverage by the University. To request a certificate of insurance, a written request should be sent to the Office of Risk Management. This request should include the name and address of the party requesting it, the purpose of the University’s use, and the dates for which coverage is required. Certificates from Other Parties
When others are contractually required to provide the University with a certificate of insurance, a copy of the certificate should be sent to the relevant department and to the Office of Contract Compliance.
Memorandum of Insurance
Vendors and third-party certificate requesters are now able to access evidence of insurance for The University of Akron in the form of a Memorandum of Insurance (MOI) located on a secured website maintained by our broker. The MOI eliminates the substantial paperwork and workflow involved in providing paper Certificates of Insurance thereby improving business efficiency.
The University will send a paper certificate of insurance to a certificate holder if they are unable to accept the MOI.
Auto ID Card
An Auto ID Card provides evidence of Auto Liability Insurance and should be located in the glove box of each University owned/leased vehicle.
