Before completing your application, please carefully read the information provided here.

How to Complete the Application Using Your Computer
  1. Open the student teaching application. To enter information, click on the line or in the box for each item and type. You may also turn on the gridlines to show the invisible tables (Table: Gridlines). The gridlines will not print.
  2. For best results, use Arial 10pt font for the application.
  3. Only type in the gray spaces provided. Click in each space to start typing.
  4. Remember to save frequently! Since it is a template, the first save should be a "Save As." Be sure to save it as a Microsoft Word document and give it a new name.
  5. Be sure to run a spell check to correct any typographical errors.
  6. Print the completed application.
  7. Take it to your advisor 2-3 weeks before the due date for his/her review and signature.
  8. Turn the completed, signed application in to the Office of Extended Educational Experience (Zook 207) by the deadline.

Due Dates for Applications
  • Application for Fall Semester Student Teaching - March 1 (or following Monday if this date falls on a weekend)
  • Application for Spring Semester Student Teaching - October 1 (or following Monday if this date falls on a weekend)

Eligibility Requirements

You must have a 2.5 overall grade point average; and a 2.5 grade point average in your teaching field, in education, and in the core courses. It is your responsibility to meet with your advisor to be sure all prerequisite courses, as required by your department, are completed prior to student teaching.


Length of Assignment Under the Semester System

There are four schedules for those students taking student teaching: (a) one 11-week, (b) two 8-week, (c) one 16-week, and (d) two 6-week. Follow this link to see which schedule applies to your program.

A meeting for student teachers for the semester is always held during the first week of each semester. ALL students are required to attend this meeting. You will be advised by letter from the Director of Extended Educational Experiences as to the specific date, time, and place of this meeting. This letter will be sent to you approximately one week prior to the beginning of the semester.

E-mail will be used to notify student teachers of important dates and information. It is essential that students provide e-mail addresses and check messages regularly throughout the semester.


HELPFUL THINGS TO KNOW

1. Grade Point
  1. The total number of accumulative college credit hours refer to a total of those semester hours the applicant has completed as of the last full semester of attendance. It does not include any hours for which the student is registered during the semester the student teaching application is submitted. The Grade Point Average can be secured from the grade report the student received at the end of the last full semester of attendance.
  2. The total number of credit hours in your major refers to all courses taken in your major teaching field, EXCLUDING all Core Courses (5050). To compute your Grade Point Average in your major teaching field multiply the credit hours of the course by the grade in each course (A = 4.00, A- = 3.70, B+ = 3.30, B = 3.00, B-= 2.70, C+ = 2.30, C = 2.00, C- = 1.70, D+ = 1.30, D = 1.00, D- = 0.70, F = 0.00. Divide the total by the total credit hours to get your grade point average in your major teaching field.

2. Location Request

Many factors are involved in providing student teaching placements in the schools. In order to assure you an optimum student teaching experience, all student teaching assignments will be determined by the Office of Extended Educational Experience. Priority will be given to our student teaching/professional development school sites in Coventry, Springfield, Stow, and Wadsworth. Other assignments will be made at the discretion of the Office of Extended Educational Experience, using any geographic preference information you provide.

You may request a geographic area of your preference; however, there can be no assurance that your application will be sent to one of these districts, or that you will, or can, be assigned to the district of your preference. Do not request a school district in which your experience may be compromised by pre-existing relationships with other personnel or students who attend the school. Students will not be placed in the district in which they attended school.

4. Advisor's Signature
It is required that your advisor review and sign your application before you submit it to the Office of Extended Educational Experience.


Checklist for turning in the Application
Before turning in your Student Teaching Application to the Educational Field Experiences Office, please be sure all necessary forms are completed and present:
  • First and second page of Student Teaching Application signed by advisor (no extra copies needed)
  • 6 Xeroxed copies of Personal Data Form
  • 6 Xeroxed copies of Course Summary (staple each Course Summary Sheet behind each Personal Data Form)
  • 6 Xeroxed copies of Reflective Essay
  • 6 Xeroxed copies of the Placement Form; fill out only the lines that have an asterisk (both top and bottom). Do not put in dates, supervising teacher, school, etc. Only complete the lines with the * before them. The secretary in the Office of Extended Educational Experiences will complete the rest of the form for you.
  • Career Placement Services form—no copies required. They have asked that you fill out and return this form to our office with your application. We will forward it over to their Career Placement.
  • Copy of Praxis II Content Area Test(s) Scores
  • Copy of Portfolio Score Sheet

Please do not list nicknames on your application. Should your name, address, or phone number change at any time between the time you submit your application and the time you finish your student teaching, please notify the Office of Student Teaching and Field Experiences of that change.

For easier processing, please keep above copies in this order when turning in. Paper clip each original page with its copies behind it, except for the Personal Data and Course Summary sheets, which should be stapled together.

Return this application in person to Zook Hall Room 207 after approval by your advisor. Be sure to include all the necessary xeroxed copies and an official copy of your transcript.
