Gradaute Student and Organization Documents

Graduate Student Government (GSG) aims to provide graduate students and organizations with a central clearinghouse for all documents and applications related to funding travel and professional development opportunities.

2009-2010 EAF Budget and Request Forms

According to the 2009-2010 EAF Guidelines provided below all of the following documents must be completed in order for registered graduate organizations to receive or be considered for funding:

GSG EAF Manual 2009-2010

Organization presidents should fill out this form annually to give GSG a glimpse of the estimated budget for the upcoming academic year. This form is intended to give GSG and idea of how to budget the annual allocation between graduate groups, individual students, and campus initiatives:

Annual Proposed Budget Form

This is the form organizations will complete with information about budget requests for the Fall/Spring semester(s). This form will be used to determine funding allocations for the semester:

Semester Budget Form Spring 2010

This form must be filled out for every event or program listed on the Semester Budget Form. It should include a detailed description of the purpose of the event and how it relates to the mission of the organization and its membership. Failure to complete this form for every event may result in denied or limited funding for the event.

The following form must be completed if bringing a contractor or speaker to an event on campus:

Graduate organizations looking to apply for a University authorized Visa card for spending money and ease of transaction should complete the following application:

Travel Authorization Forms

This form must be completed by only registered graduate student organizations that wish to travel. Group travel must consist of 2+ members of the organization. No individuals can travel on behalf of an organization. This form must be completed and submitted to the Graduate Student Government office, at least four weeks prior to incurring any travel expense.

Any requests for reimbursement after travel is complete must be submitted within two weeks after completion of travel:

This newly created program will allow individual graduate students the opportunity to travel to professional development experiences, programs, & conferences. We recognize that a much larger graduate population exists than is represented by graduate student organizations.

This document sets parameters for individuals traveling and they can receive up to $150 for travel expenses to participate in a conference and individuals presenting at a conference can receive up to $300. This form must be completed and submitted to the Graduate Student Government office, at least four weeks prior to incurring any travel expense. If you do not complete this form prior to travel, you will not receive financial assistance for any costs incurred.

Applications for Graduation

All graduate students must complete an Advancement to Candidacy/Graduation Application form to be considered for graduation. The purpose of the form is to inform the student of his remaining requirements and to notify the Registrar of the student’s intent to graduate. A student cannot graduate without having completed and submitted this form. The form is due to the Graduate School by the following dates:

Spring Graduation: September 15
Summer Graduation: February 15
Fall Graduation: May 15
