The University of Akron is pleased to continue to provide a high quality group-insurance package for full-time faculty and staff members.
The University allows employees to tailor their benefit needs. You have your choice among the following coverages:
- medical;
- dental;
- life insurance;
- long-term and short-term disability;
- vision;
- flexible spending accounts; and
- dependent life insurance.
You will find information about each of these coverages on this Web site. Use the menu to the left to locate the information you want.
A few of the links on this Web site lead to PDF documents, which require Adobe Reader software to view. If that software is not on your computer already, you can download it here for free.
COVERAGE COMMITMENT
A coverage commitment will be in place from the date of hire until Dec. 31, 2006, for all selected benefits except Dental and Vision. For these two elections, up to a two-year (2006-07) coverage commitment is in place.
If you have a change in family status (marriage, employment status, a change in the number of dependents, etc.), that change must be reported to Benefits Administration within 31 days. More information is here. Contact Benefits Administration at ext. 7092 with any questions.
Materials explaining in detail the comprehensive plan, PPO plan (Medical Mutual of Ohio) and the three HMO options (SummaCare, Kaiser or HomeTown) may be obtained through the medical page of this Web site.
IMPORTANT NOTES
- Please complete the Dependent Info section of your Personal Enrollment Form indicating which dependent is to be covered by each benefit plan. Personal Enrollment Forms submitted without dependent information will be returned. Incomplete forms will cause a delay in processing.
- If you are enrolling in life insurance, please complete the beneficiary card.
