Patients or enrollees who wish to request access to, a copy of, or an amendment to their protected health information should contact the University Privacy Official. Individuals who would like an accounting of the uses or disclosures of their protected health information or who wish to place restrictions on the use or disclosure of their protected health information should also direct their requests to the University Privacy Official.

Anyone who wishes to report a violation of privacy practices or who disagrees with a decision about their protected health information may file a complaint with the University's Privacy Official.
