Obtain an Enrollment Verification Request form from our website or pick one up in the lobby of Hezzleton E. Simmons Hall. Complete the requested information and attach any additional documentation that needs to accompany the verification. You may then fax the completed request form to 330-972-6097, return it in person to Hezzleton E. Simmons Hall, or mail it to:
