UA Conflict of Interest/Conflict of Commitment
Policy and Forms
Every University of Akron full-time employee is required to complete a Conflict of Interest/Conflict of Commitment Disclosure Form annually, per UA Rule 3359-11-17. These forms must also be updated should an employee's circumstances change.
This annual form is distributed by the Office of Research Services and Sponsored Programs and is due each year by April 30th.

The Annual Conflict of Interest and Conflict of Commitment Disclosure Form packet was mailed to every UA employee April 2, 2008. Each document is available below.
| Every campus unit is required to submit copies of all Conflict forms to the Internal Auditor's Office, +6203, no later than May 31, 2008. |
