Disciplinary Records
Rule 3359-42-01 Student Rights and Responsibilities
Information from disciplinary records may be released only upon the written request of the student. Where there is clear and probable danger which might result in loss of life, personal injury or property damage, the appropriate university officers may release this information without the consent of the student. Provisions shall be made for the periodic destruction of disciplinary records.
Release of Student Records
In order for any disciplinary records to be released to any parent/guardian, a FERPA waiver must be signed by the student in person at the Office of Student Judicial Affairs located in the Student Union, room 211.
In order for any disciplinary record information to be released to another college/university, the student must first obtain the clearance form from the probable place of education. Once this form is obtained with the proper information completed, then the student must submit this form to the Office of Student Judicial Affairs located in the Student Union, room 211.
FERPA Waiver
Background/Transfer Clearance Forms
The student must obtain the background/clearance form from the institution seeking the student records. Once this form is obtained, the student must send it to the Office of Student Judicial Affairs for completion. The Office of Student Judicial Affairs is located in the Student Union room 211.
