Short Term Disability

Dearborn National is the administrator of the University’s voluntary short term disability insurance policy.  Members can contact Dearborn National by calling 800-778-2281 or by e-mailing Claims_Customer_Service@dearbornnational.com.

This provides a brief description of the University’s voluntary short term disability insurance policy including weekly benefit payout and cost information.

Long Term Disability

United Healthcare Specialty Benefits is the administrator of the University’s long term disability insurance policy.  Members can contact United Healthcare Specialty Benefits by calling 866-615-8727.

This provides a brief description of the University’s long term disability insurance policy including monthly benefit payout, employee election options, and general cost information.

Life Insurance

Minnesota Life is the administrator of the University’s employee and dependents life insurance policies.

Please use these forms if you should sustain a work related injury. Completion of these forms does not assure the allowance of the claim in accordance with the Ohio Bureau of Workers Compensation.
