Meaningful Notice - 2020 Information

Instructions for requesting a distribution/exchange/transfer/loan from accounts under the plan:

  1. Participant contacts his or her plan provider or financial professional for the required product provider forms.
  2. Participant gathers supporting documentation.
  3. Participant completes Transaction Routing Request Form.
  4. Participant emails or faxes the product provider forms, supporting documentation and Transaction Routing Request Form to Plan Administration at recordkeeping@tsacg.com or FAX: 866.741.0645
  5. TSACG Plan contacts the participant if additional documentation is needed or the request is ineligible per the plan.
  6. If the request is approved, TSACG Plan sends the forms to the appropriate product provider(s).
