Declaration & Certification of Finances for ALL students

This form provides the estimated cost to attend UA for one academic year. Submit the completed form when you apply for your I-20 or DS-2019. You can upload in your portal or email it to us.

  • The financial statement (DCF) is not just a check list item. It is documentation that you can support yourself for at least the first two semesters that you are here, and that you have a financial plan for the balance of your studies.
  • You may need to provide the same documents at your visa appointment.
  • Please choose the correct level of funding for your planned program of study.
  • Students who receive conditional admission to the English Language Institute should select the ELI funding line on this form, not their planned academic program.
  • Instructions and requirements for supplemental documents are listed on the form.

Note: We cannot process your I-20 without a signed DCF, even if you have a scholarship or a graduate assistantship.

Please give us at least 5 business days (more during peak times) to process your I-20.

Dependent I-20 or DS-2019 Application Submit this form if you want to apply for a Certificate of Eligibility, so your spouse or child(ren) can apply for an F-2 or J-2 visa to come to the U.S. 10 business days

Invitation Letter Template If you are requesting a tourist visa for someone you want to visit you while you are in the United States.
Leaving the USA, notice for F-1/J-1 students If you have withdrawn from your program at UA and will be leaving the U.S., please complete this form to let us know.NOTE: You do not need to report if you are leaving for vacation.
Reduced Course Load Request If you are planning on taking class part-time, you will need to complete this form. Check your eligibility before you apply.

Social Security Number: Instructions for Applying If you just got a job, read this information: To start the process of obtaining an SSN, submit the employer’s letter described in this packet to the International Center.
Sample letter from the on-campus employer Submit no sooner than 30 days before the start date at the on-campus job. You must have a job offer to be eligible for an SSN. Your SEVIS record must be activated. International Center issues letter: 10 business days. Social Security Administration will send the Social Security card by mail after you submit a complete application to SSA (check with SSA for processing times).
Transfer-out Request Submit this form if you wish to transfer to another U.S. college or university. 10 business days
Travel Form for Continuing F-1 Students and J-1 Exchange Visitors Use this form to report your international travel plans and/or request a travel signature on your I-20 or DS-2019. Do not complete this form if you are a new student that has not yet started your program at UA. 3 business days

Curricular Practical Training Application

Read this packet and complete the application if you want to do an internship, co-op, off-campus practicum, or other practical training prior to graduation.

Submit at least 10 business days before intended start date of practical training.

J-1 Academic Training Packet Academic Training (AT) is off-campus employment, training, or experience in a J-1 student’s major field of study.
OPT: Apply

An eligible F-1 student may request Optional Practical Training to be employed in a position that is directly related to the major area of study.

Earliest you may apply to USCIS for OPT is 90 days before program end date.

International Center can typically schedule an appointment the same week or the next week. In most cases, your I-20 with OPT recommendation will be issued during your appointment.

You must submit your application and all supporting materials to U.S. Citizenship and Immigration Services (USCIS).

OPT: Application for STEM Extension For STEM major students, you can apply for an OPT extension. Use this form to start that process.

International Center issues the I-20 with OPT STEM Extension recommendation in 10 business days.

For students on initial 12-month post-completion OPT, use this form when your U.S. address, phone number or employment changes.

OPT: STEM Reporting Form If you are on STEM OPT, you will have to report new employment, but must also report every 6 months even if your employment hasn't changed. 10 business days
Off-campus Employment Authorization Due to Economic Hardship If you want to apply for off-campus employment due to economic hardship, fill out this form.

For students that qualify, International Center processing time is 10 business days.
